Acknowledgment of Risk Form
This form must be signed by each participant or his/ her guardian and returned to our office no less than 45 days prior to trip departure. No faxes or copies please, only original signatures will be accepted.
Beverage Order Form
If you would like to order beverages for your trip, other than those provided, please fill out this form and mail/fax to Cliff Dwellers Lodge no less than 3 weeks prior to your trip. Please be sure to fill out this form thoroughly.
